Let us start with the basics.
No pressure — rough notes work great.
Tell us about yourself.
Bullet notes are perfect — we will write the polished copy later.
Your family.
Tell us a little about yourselves — bullet notes are perfect for the parent facts.
Written in your own words — we will keep this letter verbatim on your site.
Adoptive Parent 1
Adoptive Parent 2 (optional — leave blank if single parent)
Athlete basics, eligibility, and academics.
Coach-facing fields come first — grad year, sport, position. Optional detail is at the bottom.
Coach-facing essentials
Graduation year is the #1 filter coaches use. Get this and sport/position right.
NCAA Eligibility Center
Starting the 2026-27 academic year, the NCAA Eligibility Center is being applied to all divisions, including Division III (previously D1 / D2 only). Register and check current requirements at eligibilitycenter.org. NCAA rules change — this form only collects what you have today; it does not assert or guarantee eligibility status.
Academics
The NCAA recalculates GPA from roughly 16 approved core courses (English, math, science, social science, etc.). Some coaches request this separately from your overall GPA.
Optional detail
Skippable — leave blank if they do not apply.
Pages and vibe.
Pick everything you would like — we will scope it together.
Your home and pets.
Photos help birth parents picture life with your family. Notes are optional but helpful.

Stats, measurables, and recruiting goals.
Lead with the numbers that show how you perform.
Key stats and achievements
Sport-specific bullet notes. This is the first thing coaches scan.
Measurables — verified vs self-reported
Mark each measurable as verified (combine, showcase, or third-party tested) or self-reported. Coaches scrutinize unverified numbers more carefully — being upfront builds trust. Leave any row blank if it does not apply to your sport.
Upcoming schedule — where coaches can see you
Upcoming games, showcases, camps, or tournaments where a coach could watch you play in person. Dates and locations help.
Recruiting goals
e.g. D1, D2, D3, NAIA, JUCO — comma-separate.
Any links we should know about?
Totally optional — existing site, Facebook, Instagram, whatever you have.
Everyday moments and logistics.
Snapshots of daily life, plus a few details about your adoption journey.
Everyday moments
Pancakes on Sunday, hiking, a quiet evening with the dog — add a few photos and a short description for each.
Drag photos here or
No photos yet. Add a description to each one.
Adoption agency
Fundraising link (optional)
GoFundMe or GiveSendGo. Your site will display this link prominently and track clicks/visits so you can see how it is performing.
Adoption contact info (only if different from your personal contact)
Some families use a separate email and phone for adoption inquiries. Leave blank to reuse the email and phone from Step 1.
Anything else?
Highlight video, coaches, references, and contact.
This is the page coaches share around — make every link easy to find.
Primary highlight video
Your single most important reel — the one a coach should watch first. We will embed this prominently above the fold on your site.
Additional highlight media
Anything else you want a coach to see beyond the primary reel.
High school team & coach
Club / Travel Team (if applicable)
Character references — beyond coaches
Trainers, teachers, mentors, employers — non-coach references who can speak to your work ethic and character. Two or three is plenty.
Social & online presence
Your built site is the central hub — put it in every social bio so coaches can land here.
Contact
Athlete statement
Written in the athlete's own words — we will keep this verbatim on the site.
What is your color style?
Pick a palette that feels right for your brand.
Any photos you would like to share?
Drag in a few favorites — storefront, team, products, family. We recommend 3–5. Totally optional.
Headshots and family portraits.
Photos of your family — headshots, portraits, or anything else that did not fit earlier. We will pair these with what you have already uploaded.
Photos for your profile.
A clean headshot, a few action shots, and team pictures — these are what coaches see first.
